I understand that this Release and Waiver of Liability executed on this date releases Chinese Community Center, a  501(c)(3) nonprofit organization organized and existing under the laws of the State of Texas, and each of its directors, officers, employees, and agents. I desire to provide volunteer services for Chinese Community Center and engage in activities related to serving as a volunteer.

I understand that the scope of the my relationship with Chinese Community Center is limited to a volunteer position and that no compensation is expected in return for my services provided; that Chinese Community Center will not provide any benefits traditionally associated with employment; and that I am responsible for my own insurance coverage in the event of personal injury or illness as a result of my service to Chinese Community Center. I do hereby freely,  voluntarily and without duress execute this Release of Waiver under the following terms: 

1. Waiver and Release: I release and forever discharge and hold harmless Chinese Community Center and its  successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in  law or in equity, which arise or may hereafter arise from the services I provide to Chinese Community Center. I  understand and acknowledge that this Release discharges Chinese Community Center from any liability or claim  that I may have against Chinese Community Center with respect to bodily injury, personal injury, illness, death,  or property damage that may result my work for Chinese Community Center
2. Medical Treatment: Except as otherwise agreed to by Chinese Community Center in writing, I do hereby release  and forever discharge Chinese Community Center from any claim whatsoever that arises or may hereafter arise on account of any first aid, treatment or service rendered in connection with my work for Chinese Community Center.
3. Assumption of Risk: I understand that the services I provide to Chinese Community Center may include activities that may be hazardous to me working at the Chinese Community Center. As a volunteer, I hereby expressly assume risk of injury or  harm from these activities and Release Chinese Community Center from all liability.
4. Insurance: I understand that Chinese Community Center does not carry or maintain health, accident, liability  (including without limitation motor vehicle liability), property loss or damage (including without limitation  motor vehicle collision damage), medical or disability insurance coverage for any Volunteer or the  property of any Volunteer. Each Volunteer is expected to obtain his or her own automobile, medical or  health insurance coverage. I expressly waive any such claim for compensation or liability on the part of  Chinese Community Center.
5. Photographic Release: I do hereby grant and convey unto Chinese Community Center all right, title and interest  in any and all photographic images, video or audio recordings made by Chinese Community Center during the my  work for Chinese Community Center, including, but not limited to any royalties, proceeds or other benefits derived from such photographs or recordings.
6. PII Confidentiality & Non-Disclosure: The Volunteer agrees to maintain the confidentiality of Personally Identifiable Information (PII) (information that can be used to distinguish or trace an individual’s identity, either alone or when combined with other information that is linked or linkable to a specific individual.) The Volunteer agrees not to disclose any PII obtained during their service to any third party unless required by law. Any violation of this Agreement may result in disciplinary action, termination of the Volunteer's service, and legal consequences. This Agreement shall remain in effect for the duration of the Volunteer's service with CCC and shall survive termination or completion of such service.
